MUSKEGON – WellCentive Registry has been selected for use by Wexford-Mercy PHO, a Cadillac.-based physician hospital organization affiliated with Mercy Hospital in Cadillac. The organization plans to use the system to improve clinical quality outcomes and streamline the care delivery process.
Wexford-Mercy PHO physicians will use the WellCentive Registry at the point of care as well as for patient population management. The physicians will also use the WellCentive Registry for preventive care and chronic disease management initiatives.
Through the use of the WellCentive Registry, physicians and other health care providers are better able to track patients who have chronic health conditions such as diabetes, asthma or heart disease. The WellCentive Registry also includes highly effective patient outreach capabilities, including an integrated automated telephone appointment and care reminder system called WellCentive Outreach.
